Evaluation of resistance to colistin among Klebsiella pneumoniae and Acinetobacter baumannii isolates from ICU patients in two major Tehran hospitals

چکیده
Objective: Increasing antibiotic resistance has become a major issue as a global public health, with a reported 10 million deaths per year by 2025(1). The increased resistance to colistin as a last resort treatment option among carbapenem-resistant infections causes a global public health issue in treating some major nosocomial infections such as Klebsiella pneumoniae and Acinetobacter baumannii (2). So, this study aims to evaluate the resistance rate to colistin among Acinetobacter baumannii and Klebsiella pneumoniae infections of ICU-admitted patients Methods: In this study, a total of 290 specimens were collected from blood and respiratory infections of intensive care unit (ICU) patients admitted to Imam Khomeini and Dr. Shariaty hospitals in Tehran. Molecular and biochemical identification methods were employed to identified the isolates of A.baumannii and K. pneumoniae. The Minimum Inhibitory Concentration (MIC) method (3) was utilized to assess the colistin resistance patterns against these two infections. Results: The proportion of A.baumannii and K.pneumoniae were 16.89% (49/290) and 42.75% (124/290) respectively. Also, the colistin resistance against A.baumannii and K.pneumoniae were 12.24% (4/49) and 15.32% (19/124) respectively. Conclusion: This study highlights that, while resistance to colistin is not currently a significant concern, the need to use non-polymyxin multidrug therapies under international guidelines for colistin therapy is emphasized to reduce the incidence of resistance to this drug among hospital-acquired infections. .
